The Care Choices: Inpatient Palliative Care Team is an inter-disciplinary team that assesses and helps to manage patient's pain and other symptoms. They provide support and discuss treatment preferences with patients and their families to help improve patient's and family's quality of life.

Outpatient Palliative Care Clinic

An outpatient clinic for members who have progressive life limiting medical conditions such as cancer, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), chronic kidney disease (CKD) and have been referred to this clinic by their physician.

A team of professionals including a physician, a nurse and a social worker meet with members and their families/caregivers in an initial 60-90 minute office visit. This visit may include a physician's examination, a consultation for education about the member's illness, a discussion of immediate and long-term concerns and the creation of a plan of care and follow-up.

• Set goals of treatment and follow-up
• Discuss treatment preferences and advance directives/POLST
• Provide needed support and resources
